  5. Meet state certification requirements

    by meeting with Cindy Peckenpaugh to discuss the binder
    Criteria
    your supervised teaching binder contains documentation of 14 theory hours
    your supervised teaching binder contains documentation of 14 practical hours
    your supervised teaching binder contains documentation of 14 client service hours
    your supervised teaching binder includes learning plans for each class you have taught
    your supervised teaching binder includes coordinating instructor evaluations sheets for each class you taught
    your supervised teaching binder includes student evaluation feedback from your students for each class you taught
    your supervised teaching binder includes all worksheets and quizzes for each class you taught
    your supervised teaching  binder includes 3-tests; (1) T/F, (1)  multiple choice and (1) essay
    you complete your supervised teaching by 8-2-2013
    o    your supervised teaching evaluation assessment form is 75% or above for each class you taught
